“With the realization that many overdoses attributed to heroin are actually caused by fentanyl, this presentation will give the audience insight into the reasons that fentanyl is used as a substitute for heroin on the black market, and it will highlight how the very properties that make fentanyl an appropriate choice for anesthesia and end-of-life pain also make it an a deadly substitute for heroin. The history of this drug will be traced from its synthesis by Dr. Paul Janssen in 1960 to its illegal synthesis by George Marquardt, a high school dropout and self-taught chemist, and on to its role in today’s drug abuse epidemic. The pharmacology of prescription versions of the drug will be examined along with necessary cautions about its use in medical practice. Next the potency of fentanyl compared to other opioids as well as more potent congeners of itself will be discussed. Lastly, the effect of this increased potency will then be discussed in relation to overdose rescue attempts and the need for multiple doses of naloxone in such situations.” – MKCM National Case Management
