In terms of the voucher, when sending a QME or AME letter, also include the voucher form (Form DWC-AD 10133.36). This way, the doctor is actually provided the form and if they don’t fill it out, the defendant has an argument to not provide the voucher, since defendant provided all of the documentation needed, but the doctor and Applicant did not follow through with obtaining same, i.e. the defendant didn’t have the information upon which to issue the voucher.
